ADDITIONAL INFORMATION:

You may be asked to complete this form by a victim of crime or as

provider of emergency first aid or emergency health care (e.g. fire,

police or ambulance), who believes they were exposed to Hepatitis

B, Hepatitis C or HIV. The victim may ask the medical officer of

​health to order that blood be taken from the individual believed to

be the source of the exposure (the respondent), even without

consent from that individual. Further information regarding

the details and obligations of the patient and physician can be found

here, or by going to the OMA website www.oma.org and searching

“Bill 105”.

The patient takes both completed forms to their local Medical Officer

of Health who issues a subpoena for mandatory testing of the source

​patient.

You can bill a K031 for completing the physician form.
